is a key part of both towns; we aim to provide the highest standard of care to our patients and families through a quality, safe and patient centred approach. We are supported by our Patient Participation Group, who aim to act as facilitators between patients and the GP Practice, communicating interests, concerns, experiences and vital feedback. The Core PPG group are a collective of patients from both Riverside and Tower House, who meet with the Practice Manager and clinical representatives every two months. Health and Wellbeing Team

We have a wealth of informed staff at Riverside and Tower House in our Health and Wellbeing Team which comprise of Social Prescribing Link Workers and a Health Coach, and these roles are key, non-medical components of the NHS Universal Personalised Care Model, which enables patients to access the support that is needed, when it is needed. The team are highly skilled individuals who have completed accredited training with the Personalised Care Institute in line with their role. In addition to this, they are trained in Mental Health First Aid and Applied Suicide Intervention Skills. The team offer a holistic, non-judgemental approach which starts with a conversation about ‘what matters to you’, as opposed to ‘what is the matter with you’; this conversation is the start of a journey to empower patients to take control and improve their overall health and wellbeing.

There are many things which can affect our health and wellbeing and the team can work with patients who may find themselves in a situation and not know where to turn. Referrals to the team can be for support including, but not limited to problems such as:

  • Isolation and loneliness
  • Low to moderate anxiety and/or depression which interferes with daily life
  • Long term health conditions
  • Caring responsibilities
  • Financial Hardship or debt
  • Employment or unemployment
  • Volunteering
  • Housing
  • Family and relationship issues

Social Prescribing Link Workers connect people to activities, groups and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing.  After an initial assessment which lasts around an hour, they will offer a series of follow-up appointments. On average, patients have between 4-6 contacts with their Social Prescribing Link Worker.

Health Coaches take people through 6 structured sessions using goal setting techniques to identify something that they want to change in order to improve their health and wellbeing; this can be particularly effective for patients who lack motivation to make positive change.
